Review the present perfect structure (has/have + past participle) and remind students that we use the present perfect to talk about actions from the past that have some relevance in the present or continue into the present day (are not completed) and when we don't know or don't care about the specific time in the past they happened.
Teach the students that "For" is used to give a duration of time, and "Since" is used to indicate the start date of the action. Give some examples to illustrate this "I have studied English since I was 5" "I have studied English for 10 years"...(and I still study English). Show the difference if the action has finished by now: "I studied English for 10 years"... (but I don't study it anymore).
Have the students complete the worksheet and check their answers. Have them read the sentences out loud and give extra examples if anyone is still unsure of how "for" and "since" are used.

10mins - Review Activity: Bucket list. Have each student create a list of 10 things they would like to do in their life, or have already done. (It should be a mix of the two, and it should be secret!) Next, have students ask each other questions about their lists, using the present perfect form. (Have you ever.../ Yes, I have / No, I haven't)
